UIL Regional Event: Form 1 Eligibility Certification
The Form 1 Eligibility Certification is required for every UIL Music event in which you plan to enter students. The form must be printed and signed by your principal or superintendent, and must be turned in to your Region Executive Secretary either on or immediately before the day of the event.
Failure to submit this form may result in your students being ineligible to participate in UIL regional events. Be sure to complete and submit it on time to avoid any issues.
To complete the form:
- Sign in to your account at texasmusicforms.com and click on the Form 1 tile.
- The school's TEA code should auto-populate, but if not, you will need to enter it. The Texas Education Agency (T.E.A.) code is required for all participating entries. If you don't know what your TEA code is, contact your Region Executive Secretary. Entering an incorrect TEA code could negatively impact your entries.
Select the Event for which Form 1 is being completed from the drop-down.
Select the Organization Event from the drop-down. You will need to select the contest event first
Select the Performing Group Type from the drop-down
- Enter your student(s) names -
Enter your performing students in alphabetical order (last name, first name), each on a separate line as in the example below (use the enter or return key).
For the Solo and Ensemble contest, you can click the button below to import your student names from your existing S & E entries.
- You can type the names in the box, or you can prepare your list in another program and copy and paste it into this box.
- You can leave this blank and attach a separate list of students to your Form 1.
Once completed, click Submit and View Form. This will open the form in a new tab where you can print it for a signature. Once the form is signed, it should be delivered to the Region Executive Secretary on the day of the event or during the week prior.
