UIL Regional Event: Form 1 Eligibility Certification

The Form 1 Eligibility Certification is required for every event you plan to enter students in.


The form needs to be printed out and signed by your principal or superintendent. You can then mail the form to the executive secretary along with payment for the event you are registering students for. Fees will be listed on Forms 2-8 as you fill them out for your registrants.


Failure to complete this form may prevent you from being able to participate in UIL regional events.


To complete the form:

  1. Enter your schools TEA Code. The Texas Education Agency (T.E.A.) code is now required for all participating entries. If you don't know what your TEA code is, contact your Regional Secretary. Entering the incorrect TEA code could negatively impact your entries.
  2. Select the Event the Form 1 is being completed for from the drop down.

  3. Select the Organization Event from the drop down. You will need to select the contest event first

  4. Select the Performing Group Type from the drop down

  5. Enter your student(s) names -
    1. Enter your performing students in alphabetical order (last name, first name) each on a separate line as in the example below (use the enter or return key). 

    2. You can also click the button below to import your student names from existing S & E entries.

    3. You can prepare your list in another program, then copy and paste into this box
    4. Or you can leave this blank and attach a separate list of students to the Form 1.
  6. Submit and View Form - This will open the form in a new tab where you can print it for signature. Once the form is signed, it should be mailed to the Regional Secretary with payment for the UIL event. These fees will be listed on the Form 2-8 as you fill them out for your entrants for the events
