UIL Regional Event: Form 4/5 Concert & Sight-Reading

The Form 4/5 Concert & Sight-Reading forms are the entry forms to participate in the Concert & Sight-Reading Events. The events are set up by the Regional Secretaries and are specific to your Region and Conference.

NOTE: It is very important that you verify that you have the correct Region and Performance Conference set on your Profile. If those are incorrect, your band will be entered in the incorrect events and it also will impact your ability to advance.

To fill out the form(s):

  1. Click the Form 4/5 Concert & Sight-Reading tab on the left

  2. Fill in the T.E.A Code. This is a mandatory field. If you are not sure what your TEA code is, check with your Regional Secretary or look it up here. Don't guess at this as it could negatively impact your entries.

    In Section 1 Enter Performance Information:

    1. Select the event you wish to register for from the drop down. Events are listed by Event Date - Soft Deadline (date which late fees will be applied) - Hard deadline (date which entries are no longer permitted)

      NOTE: If you don't see your event, check your profile page and make sure you have the correct Region and Performance Conference selected. If you still don't see it, contact your Regional Secretary and validate the event name.

    2. Select the Organization Event from the drop down. Only events for your Conference (listed to the right) will be displayed

    3. Select the Performing Group Type from the drop down.

      NOTE: Fees are set up by performing group type

    4. Enter the number of students that will be entered for this event.

      NOTE: You may have multiple groups participating - each should have their own entry.

    5. Enter any additional directors associated with the entry

    6. Enter the accompanist if there is one

Once you have Section 1 filled in, you will need to fill in Section 2

  1. In Section 2 Enter Performance Selections

    1. Fill in the Sightreading Voicing. The voicing refers to soprano, alto, tenor or bass voices.

      Note: It is important to fill this in as this information is used by the Regional Secretaries to determine how many copies of the various vocal sight-reading pieces that will be needed for the competition.

      If you are unsure of requirements for the event, click the grey "PML Performance Requirements" button. This will open a new tab for your review.


    2. Make a selection for Sightreading Level from the drop down. If you are unsure what to fill in here, click the hyperlink which will take you to the official UIL site for directions.

      Make selections from the PML by clicking the blue "Select From PML" button.

      1. This will open a new window that will allow you to select up to three selections for your entry.
        1. When the window opens, select the UIL Event from the drop down

        2. Select the Grade level of the items you would like to pick from

        3. All of the eligible titles that meet the criteria you selected above will be displayed

          Example:

        4. If you know the piece(s) you want to select, you can enter it in the search box and the results listed will be reduced to match your search terms

        5. If you are not sure what piece(s) you want to select, you can scroll through the page(s).

          You can also adjust how many titles per page you would like to see.

        6. Once you have found the Title you want, click the "Thumbs up" icon. This will automatically populate the selection in the entry form for you.

          NOTE: You can order each of the columns by clicking the "arrow" at the top of the column

          NOTE: Click the "book" icon to see the publisher of the piece in a pop up

          NOTE: If you don't se the title you are looking for here, it is not on the official Prescribed Music List.

        7. When you have finished making your selections, close the window. The entries you have selected will automatically populate to your entry form.

          8. You can click the black "Clear" button next to any selection to remove it and pick something else.

          9. If the title will be performed A Cappella, change the slider to "Yes"

          10. Fill in the "Voicing" field. The voicing refers to soprano, alto, tenor or bass voices.


      2. Click the green "Submit & View C/SR Entry

      3. The Rating sheet will open in a new tab and can be printed from there. Validate that all of the information is correct. If it isn't, modify the entry or delete it and enter it again.

      4. To print the rating sheet for Sight-Reading, navigate to your "My Entries" panel. (Directions can be found here.)
