UIL Regional Event: Form 4/5 Concert & Sight-Reading
The Form 4/5 Concert & Sight-Reading forms are the entry forms needed to participate in Concert & Sight-Reading Events. The events are set up by the Region Secretaries and are specific to your Region and Conference.
NOTE: You must verify that you have the correct Region and Performance Conference set on your Profile. If incorrect, you will be unable to see the correct events.
To fill out the form(s):
Click the Form 4/5 Concert & Sight-Reading tab on the left.
The T.E.A. Code should auto-populate, but if not, you will need to fill it in as it is a mandatory field. If you are unsure what your TEA code is, check with your Region Secretary or look it up here. Don't guess, as it could negatively impact your entries.
In Section 1, Enter Performance Information:
Select the event you wish to enter from the drop-down. Events are listed by Event Date - Soft Deadline (date at which late fees will be applied) - Hard deadline (date by which entries are no longer permitted). NOTE: If you don't see your event, check your profile page and make sure you have the correct Region and Performance Conference selected. If you still don't see it, contact your Region Secretary and validate the event name.
Select the Organization Event from the drop-down. Only events for your Conference (listed to the right) will be displayed.
Select the Performing Group Type from the drop-down. NOTE: Fees are set up by performing group type.
Enter the number of students that will be entered for this event. NOTE: You may have multiple groups participating - each group should have its own entry.
Enter any additional directors associated with the entry
For Choir: Enter the accompanist if there is one.
Once you have Section 1 filled in, you will need to fill in Section 2
In Section 2, Enter Performance Selections
For Choir: Fill in the Sight-Reading Voicing. The voicing refers to soprano, alto, tenor, or bass voices. Note: It is important to fill this in as this information is used by the Regional Secretaries to determine how many copies of the various vocal sight-reading pieces that will be needed for the competition. If you are unsure of the requirements for the event, click the grey "PML Performance Requirements" button. This will open a new tab for your review.
Make a selection for Sight-Reading Level from the drop-down. If you are unsure what to fill in here, click the hyperlink, which will take you to the official UIL site for directions.
Make selections from the PML by clicking the blue "Select From PML" button.
This will open a new window that will allow you to select up to three selections for your entry.
When the window opens, select the UIL Event from the drop-down
Select the Grade level of the items you would like to pick from
All of the eligible titles that meet the criteria you selected above will be displayed
If you know the piece(s) you want to select, you can enter it in the search box and the results listed will be reduced to match your search terms
If you are not sure what piece(s) you want to select, you can scroll through the page(s).
You can also adjust how many titles per page you would like to see.
Once you have found the Title you want, click the "Thumbs up" icon. This will automatically populate the selection in the entry form for you.
NOTE: You can order each of the columns by clicking the "arrow" at the top of the column
NOTE: Click the "book" icon to see the publisher of the piece in a pop up
NOTE: If you don't see the title you are looking for here, it is not on the official Prescribed Music List.
When you have finished making your selections, close the window. The entries you have selected will automatically populate your entry form.
You can click the black "Clear" button next to any selection to remove it and pick something else.
For Choir: If the title will be performed A Cappella, change the slider to "Yes"
For Choir: Fill in the "Voicing" field. The voicing refers to soprano, alto, tenor, or bass voices.
Click the green "Submit & View C/SR Entry.
The Rating sheet will open in a new tab and can be printed from there. Validate that all of the information is correct. If it isn't, modify the entry or delete it and enter it again.
To print the rating sheets after you have left the site, navigate to your "My Entries" panel. (Directions can be found here.)
